Target: BP - A Hypertension Resource for Improving Care Quality, Outcomes, and Health Center Recognition
Workshop Overview
Target: BP™ is a national initiative created by the American Heart Association (AHA) and the American Medical Association (AMA) in response to the high prevalence of uncontrolled blood pressure (BP). Committed to advancing positive health outcomes, Target: BP supports health care organizations and communities to improve blood pressure control by:1) Leveraging AHA guidelines and scientific statements and the AMA MAP™ framework; 2) Assisting health care organizations in their quality improvement (QI) journeys to improve and sustain BP control; 3) Recognizing health care organizations annually with achievement awards
This workshop would provide an overview of the key concepts of the AMA MAP framework as an approach to hypertension QI including measuring accurately, acting rapidly, and partnering with patients. Available resources associated with each aspect of the framework will be highlighted to strengthen professional competency, systematic care processes, and culturally relevant patient education efforts. In addition, the structure, criteria, process, and benefits of submitting data for annual achievement awards will be discussed using aggregated performance data from the 839 FQHCs currently participating in Target: BP.
The award criteria emphasize the adoption of evidence-based activities associated with the MAP framework, including self-measured blood pressure monitoring and equitable health outcomes. Participants can use assessment tools and the award criteria to evaluate their current practices, identify gaps, choose improvement resources, and take action to improve blood pressure control. Examples will be shared illustrating how annual awards can be used to incentivize improvement, celebrate team-based efforts, and promote organizational excellence with patient and community members.

Roundtable Discussion: Dual FQHCs and CCBHCs – Successes, Challenges, and Lessons Learned (Invitation Only)
Session Overview
RSVP required. Nationally, there are approximately 60 dual Federally Qualified Health Centers (FQHCs including Look-a-Likes) who are also Certified Community Behavioral Health Clinics. With other Community Health Center organizations that are actively pursuing a CCBHC certification. When operating and maintaining both models, organizations must determine the scope of services, finances, workforce, and data sharing, among other administrative and clinical considerations, under one roof.
Join us for a dynamic and collaborative two-hour roundtable discussion designed specifically for FQHCs who are OR are actively seeking dual certification with the CCBHC program. This session will provide a unique opportunity for FQHC leaders to come together to:
- Share valuable lessons learned from experiences integrating primary care and behavioral health in these two settings.
- Discuss key challenges and opportunities related to dual certification under HRSA and SAMHSA and explore strategies for navigating these complexities.
- Gain actionable insights on how to streamline operations, improve patient care, and strengthen your organization’s leadership in integrated care.
- Collaborate and have fun with your peers!

- Understand the unique state and federal challenges that FQHCs encounter when seeking to become a CCBHC, aka dual certification.
- Identify strategies to merge and coordinate workflows, program requirements and billing etc. under both programs.
