Federally Qualified Health Centers (FQHCs) are uniquely positioned to address the healthcare needs of underserved communities, yet limited resources and high patient demand often challenge timely and continuous care. Remote Patient Monitoring (RPM) offers a transformative solution by connecting patients and providers beyond the traditional clinic setting, enabling proactive care and improved outcomes.
The presentation will be conducted through a panel discussion to explore how RPM can be strategically implemented within FQHCs to enhance chronic disease management, reduce hospital readmissions, and empower patients in self-care. The panel will consist of a qualified supplier (Remetrich Health), a Medical Supply Distributor (McKesson), and depending on availability and proximity to the conference an FQHC that has implemented RRM.
MedTrainer will serve as the moderator and will include a brief introduction at the end of the panel discussion on how MT provides the compliance and learning platform to support RPM and more.
