CMH2 - Creating a Culture of Continuous Quality Improvement through Sustainable Infrastructure

Session Description: Urban Health Plan (UHP) has built a long-standing infrastructure to support continuous quality improvement (CQI) that is sustained through its Institute for the Advancement of Community Health (IACH). As a discrete quality improvement institute and innovation arm of UHP, IACH’s mission is to improve the health status of underserved communities by developing and disseminating innovative best practices. Drawing on evidence-based QI models learned at the Institute for Healthcare Improvement (IHI), UHP fosters a culture of quality supported at the highest level of leadership. UHP has continuously demonstrated improvement including but not limited to diabetes control, hypertension control, depression remission, asthma control and HIV key indicators. Our process for mining data to review health disparities, stratifying populations where disparities are greatest, addressing the social drivers of health through creating interdisciplinary performance improvement teams to improve health outcomes has been sustainable for two decades. These CQI processes have not only led to improvements in quality improvement metrics but support grant acquisition and regrants. UHP will share our infrastructure, key CQI processes, and our next steps for addressing new challenges.
