CMF1 - Enhancing Medicare Patient Outcomes and Achieving Financial Success Through Workflow Redesign in a Value-Based Care Arrangement

Session Description: Since 2022, Muskingum Valley Health Center (MVHC) in Ohio has participated in the ACO REACH program through Medical Home Network’s (MHN) value-based care partnership. By adopting MHN’s team-based model of care and approach, MVHC successfully transformed its care delivery model to improve patient outcomes for Medicare beneficiaries while earning shared savings.. By redesigning workflows, optimizing care management, and leveraging data-driven decision-making, MVHC enhanced patient engagement, reduced hospitalizations, and improved chronic disease management.
MVHC leveraged key strategies including embedding care managers within primary care teams, expanding access to preventive services, and utilizing real-time analytics to identify high-risk patients. These efforts not only led to better health outcomes but also positioned MVHC as a leader in value-based care. This session will explore MVHC’s journey, detailing the operational changes, challenges, and lessons learned to guide other community health centers in achieving similar success.
