Session Description: The CHC Movement is at a time of generational shift. We were born 60 years ago and many of our early leaders, the people who created our mission, are retiring or moving on. And thankfully, there are many still among us whose roots go back to our early days.

This session’s panelists are inspirational leaders who will share stories from our roots that will guide and direct our paths into the future.

Paloma Izquierdo-Hernandez is the CEO of Urban Health Plan in New York State’s and is the current Board Chair of NACHC. Her father, Dr. Richard Izquierdo, founded Urban Health Plan in 1974 and Ms. Hernandez has been a central part of this mission for most of her life. Her NACHC leadership embraces our core values and fuses them with clear visions for moving forward.

Yvonne Davis is the Board Chair at Health Care Partners of South Carolina. She is a longtime CHC Board Member leader and has served on the NACHC Board of Directors and the Executive Committee of the NACHC BOD. Ms. Davis brings to life the difference CHC’s make in people’s lives and grounds our future plans in our core values and roots.
