Join us for a dynamic “Partner of Choice” session hosted by Dr. Kyu Rhee, featuring leading voices in the Food is Medicine movement. Building on the morning plenary by Dr. Dariush Mozaffarian, this panel will explore the strategic pathways to scale nutrition-based interventions across primary care settings. Panelists will discuss innovative care models, policy levers, and funding mechanisms that support the integration of Food is Medicine models at CHCs. With insights from Tufts University, Feeding America, and Abbott, and moderated by Ben Money, MPH, SVP of Population Health at NACHC, this session will offer a high-level dialogue and invite audience engagement around the future of nutrition in primary care.
